At Transition Community Services, our Community Participation Support (CPS) services—offered both in the community and remotely—are designed to empower individuals with intellectual and developmental disabilities to explore their interests, grow through experience, and actively contribute to their communities.

Our staff work closely alongside individuals to build on their interests and incorporate personal goals. These goals may range from improving socialization skills to pursuing volunteer roles or employment opportunities. Whether it’s learning new skills or engaging with others, our team supports individuals in real-world settings where they can thrive and develop.

We believe every person should have the opportunity to live a life of inclusion, choice, and purpose. Our CPS services are tailored to reflect each individual’s outcomes, empowering them to achieve meaningful milestones whether in-person or remotely.
